CCFP Medical Home Services

As well as family practice office care, several of our physicians provide low risk obstetrical care and newborn care in hospital. Several of our physicians provide geriatric care, both in hospital and in long term care facilities and many of our physicians perform minor surgical procedures.

We are unique in that we are a family practice that offers routine infant and childhood vaccines at our office.

We have an obstetrician-gynecologist, counsellors and a psychologist practicing in our team.

After Hours Services

CCFP provides 24/7 availability to our patients. After hours please call our main phone number ((306) 244-3016) to speak to the physician on call for urgent medical advice. CCFP offers extended hours urgent care service to our patients Monday through Thursday from 5:30 - 8:30 PM. Appointments are not required. Please do not use the after hours service for non-urgent problems or prescription refills. If you have a prescription concern you can speak to your pharmacist. If you prefer, you can also call 811, our provincial HealthLine or visit www.health.gov.sk.ca/healthline-online.

Primary Health Services

Beginning in June 2015, we are delighted to expand our "medical home" to include Nursing and Mental Health services under contract with the Saskatoon Regional Health Authority. We now have a Registered Nurse working full time in the clinic to assist with case management and expand the services provided by the physicians. We also have a Mental Health worker who is contracted to provide half time service to our patients on site. As well, Primary Health has established a satellite office on our premises, through which Healthy and Home, Client Patient Access Services, the LiveWell program and other primary care services will be offered.

Mental Health Counselling

Patients at CCFP can access our SHA-funded Mental Health Counsellor if they are experiencing mental health and/or addiction issues. Brief therapy, intervention and/or referral to appropriate services are provided.

You can access this service by requesting that the counsellor contact you for services through the front desk or through your physician. Once your request is received, The Mental Health Counsellor will contact you to arrange an appointment or address your concern.

Please be advised that this is not an emergent/crisis service.

We also host a private counselling practice. Shayna Das offers counselling group for children, teens, adults, families, and couples. Their services are covered through several insurance plans - please check with your provider.

**COVID-19** Services will be offered online using Zoom video conferencing platform until further notice. There are several free resources on her website for support at this time.

Uninsured Services

Uninsured services are services provided by a physician that are not covered by provincial health insurance (Medicare). This includes, but is not limited to, cosmetic procedures, and third party requests such as sick notes, insurance forms, school forms or other reports requested by an external agency. We will bill patients privately for these services. Our fees for these services have been developed using the Saskatchewan Medical Association Uninsured Services Schedule. You will not be charged an additional fee if the agency requesting the report covers the cost of the report.

*Please note, if you do not have a valid provincial Health Service Card, you will be expected to pay for any services you receive at the time of service. The fee for service will be in an amount equal to the fees designated by Saskatchewan Health Medical Services.

CCFP physicians offer a wide variety of procedural and consultative services to our patients. In addition we will accept referrals from other family physicians for the following:

  • Circumcision
  • Scalpel-free Vasectomy
  • Attention Deficit Disorder Consultation (children and adults)
  • Intra-Uterine Device Insertion
  • Endometrial Biopsy
  • Infant tongue-tie
  • Routine Skin Excisions and skin biopsy (note: there may be a fee if this is considered a strictly cosmetic procedure)
  • Obstetrical Care (self-referral also accepted - CCFP physicians will accept new maternity patients who do not have a Family Physician as well as those referred by their usual physician)
  • Sclerotherapy for Varicose Veins
  • Botox for cosmetic purposes and botulinum toxin for hyperhidrosis (excessive sweating)
